Implementing a COPD discharge care Bundle, Chelsea & Westminster NHS Foundation Trust

Why we think this project should take place?

To build on and sustain the work carried out in the original CLAHRC funded project April 2009 to October 2010 which successfully implemented the care bundle on the respiratory ward. The improved care and standardised practice which this bundle provides should be available to all patients admitted with an exacerbation of COPD irrespective of the location of the place in which they are treated.  

 

What are we trying to accomplish?

The project team aim to deliver the care bundle to all patients admitted with an exacerbation of COPD.

 All patients admitted to Chelsea and Westminster NHS Trust with an exacerbation of COPD should be confident that they will be consistently treated according to best practice and that this will occur irrespective of which ward they are admitted to. 

 

How we will know that a change is an improvement?

Outcome data will be continuously monitored through statistical process.

Staff surveys will be carried out pre- and post- bundle to assess staff confidence and competence to administer the elements of the bundle e.g.  inhaler technique skills/smoking cessation information

Patient satisfaction surveys will be undertaken.

We will record weekly process measures of compliance with the care bundle.

 

What changes can we make that will result in an improvement?

Embed the delivery of the bundle into everyday clinical practice for all COPD patients: 

Reinforce and support the delivery of bundle by newly recruited staff on respiratory ward with continued educational sessions on smoking cessation, inhaler technique and COPD awareness.

First introduction to staff on Acute Assessment Unit with delivery of educational sessions.
