Evaluation of a quality improvement framework to improve antibiotic prescribing in hospitals (AQIP) and Improving the trends and risk factors associated with community acquired C Diff infections

Sickle Cell Anaemia

Improving care of patients with Sickle Cell Disease in primary care

Mental Health

Making it easier for users to access services for common mental health problems and improving access to psychological services for people of ethnic minority background

Telemonitoring

Implementation of telemonitoring in the management of acute exacerbations of chronic heart failure and respiratory failure/chronic obstructive pulmonary disease.

This project is also being under-pinned by an educational workstream within the Northwest London Health Innovation and Education Cluster (HIEC)

The Collaborative Alcohol Strategy and delivery of brief and in-depth alcohol reduction interventions in Northwest London

Vascular Risk Assessment

Evaluation of Vascular Risk Assessment and Application of Best Practice in Hammersmith & Fulham PCT and Ealing PCT and the implementation of a Healthly Hearts & Minds (HHM) programme

Chronic Heart Failure

Collaborative project between Chelsea and Westminster Hospital Foundation Trust and West Middlesex University Hospital NHS Trust aimed at improving the quality of life in individuals with heart failure through development of an integrated care pathway consisting of in patient and out patient arms.
